Jami Dulaney MD Wellness Podcast: What I Got Wrong About Nutrition

Welcome! And thanks for listening. In this solo episode, I wanted to give you an update on my own health, nutrition, and endurance journey over the past several months. After more than 20 years of being plant based, I've made a major shift toward a very low-carbohydrate, animal-based diet and have been closely tracking my glucose and ketones along the way. I've also continued running ultras, which feel better than ever on this new diet. We talk about what I've learned from making this transition, why I've changed the way I think about insulin resistance and metabolic health, and why I believe doctors have to be willing to change their minds when the evidence and our own experiences challenge what we've previously believed. I also share some of the things I think people on both sides of the nutrition debate can agree on, why I believe we should look beyond headlines and study abstracts and actually examine the details of the research, and why being willing to listen to people on the "other side" can ultimately make us better clinicians and better patients. Jami Dulaney MD Wellness Podcast: Rethinking Carbs, Running & Metabolic Health with Dr. Tim Noakes

Welcome! And thanks for listening. In this episode, I have the incredible opportunity to sit down with one of my lifelong mentors in both medicine and endurance running, Dr. Tim Noakes. I first discovered his work when I started running marathons in 1999, and his books completely shaped the way I thought about endurance training, hydration, and fueling. But Dr. Noakes' story is also a story of changing your mind when the evidence and your own experience tell you that something isn't working. We talk about his evolution from promoting high-carbohydrate fueling to questioning that approach, his own experience with type 2 diabetes and metabolic health, and the research that changed the way he thinks about endurance performance. We also dive into my own journey with insulin resistance, continuous glucose monitoring, low-carbohydrate endurance training, and what happened when I began experimenting with how I fueled my ultramarathons. This is a conversation about being willing to question what you've been taught, examine your own experience, and keep learning as the science evolves. Thanks as always for listening. Jami Dulaney MD Wellness Podcast: Everything We're Taught About Diet is WRONG | with Dave Mac

Welcome! And thank you for listening. In this episode, I take a deeper look at some of the conventional ideas we’ve been taught about nutrition, carbohydrates, vegetables, meat, and metabolic health. We discuss what happens when we begin questioning the standard advice around what a healthy diet should look like, including the role of carbohydrates and insulin, the potential impact of constantly elevated glucose and insulin, and the debate surrounding plant-based versus animal-based nutrition. We also explore the importance of nutrient absorption, gut health, and metabolic health, and why sometimes the biggest changes in our health begin when we’re willing to question what we’ve always been told. Jami Dulaney MD Wellness Podcast: Is Insulin Resistance the Real Cause of Heart Disease? | With Dr. Philip Ovadia

Welcome! And thank you for listening. In this episode, I sit down with cardiothoracic surgeon Dr. Philip Ovadia to challenge some of the conventional thinking around heart disease, cholesterol, nutrition, and metabolic health. After becoming morbidly obese and pre-diabetic himself despite being a heart surgeon, Dr. Ovadia began questioning the advice he had been giving patients and ultimately transformed his own health through a low-carbohydrate, ketogenic approach. We discuss why insulin resistance may be a much bigger factor in heart disease than cholesterol alone, the role of inflammation and metabolic health, and why Dr. Ovadia has completely changed his views on dietary fat, meat, and plant-based diets. Jami Dulaney MD Wellness Podcast: Are We Looking at Heart Health All Wrong? | With Dr. Bret Scher

Welcome! And thank you for listening. In this episode, I sit down with Dr. Bret Scher. He is a board-certified cardiologist, lipidologist, and leading expert in therapeutic uses of metabolic therapies, including ketogenic diets. We discuss Dr. Scher’s journey from conventional cardiology to ketogenic medicine, the connection between insulin resistance and cardiovascular health, and why looking only at LDL cholesterol may not tell the whole story. We explore triglyceride-to-HDL ratios, fasting insulin, blood sugar, calcium scores, CT angiography, and the complex relationship between ketogenic diets and rising LDL. Finally, we discuss ketogenic therapy and mental health, metabolic flexibility, and what endurance athletes can learn about nutrition and avoiding the dreaded “bonk.” Thanks as always for listening. Jami Dulaney MD Wellness Podcast: Runner's Heart and Ketogenic Diet Update

Welcome! And thank you for listening. In this episode, I discuss the importance of recognizing potentially serious heart symptoms in runners and endurance athletes, using the story of a young runner who died unexpectedly after experiencing repeated episodes of severe palpitations. We talk about why symptoms like sudden palpitations, chest pain, and unusually high heart rates should not simply be dismissed as dehydration or anxiety, why it's important to rule out serious cardiac conditions first, and how athletes can pay closer attention to their heart rate and warning signs during exercise. I also share an update on my ketogenic diet and my experience tracking glucose and ketones, including how exercise, insulin, carbohydrate intake, and dietary fat can affect ketone levels, why endurance athletes may use ketones faster than they produce them, and why patience and personal data are important when transitioning to a ketogenic diet. As always, thank you for listening. doctordulaney.com email: jami@doctordulaney.com water distillers: https://mypurewater.com/?sld=jdulaney. discount code: cleanwaterforsophie

Jami Dulaney MD Wellness Podcast: How My Insulin Resistance Changed Everything

Welcome! And thank you for listening. Diabetes has shaped my life long before I became a cardiologist. After watching multiple family members suffer from its complications, I was determined to avoid the same fate. Despite years of endurance training and following a whole food plant-based diet, I began noticing the signs of insulin resistance in my own body. In this episode, I share why I made the decision to transition to a low carbohydrate, high saturated fat approach and the lessons I've learned along the way. This is just the beginning of my journey, and I'll be sharing my progress, setbacks, and what I learn as I work toward reversing insulin resistance and improving my metabolic health. Jami Dulaney MD Wellness Podcast: Dopamine, Diet, and Discipline

Welcome! And thank you for listening. One of the biggest barriers to lasting change is that every dietary pattern asks us to give something up. Whether it is processed carbohydrates, sweets, or other comfort foods, we have to replace the pleasure they once provided with something that better aligns with our long-term goals. Understanding your "why" is essential to creating lasting change. For me, that means improving insulin resistance, lowering inflammation, and helping people regain metabolic flexibility so their bodies can efficiently use stored fat for energy. In this episode, I discuss how dopamine influences cravings, why insulin resistance makes weight loss so difficult for many people, and why constantly eating and snacking can keep us trapped in a cycle of elevated glucose and insulin. I also share why I believe nutrition should always be individualized and why education, community, and support are critical for creating lasting change. If you would like to join us. Jami Dulaney MD Wellness Podcast: Sugar, Fiber and Fat : Which is it?

Welcome! and Thank you for listening. I have been to Alaska fishing! I am back with fresh salmon and halibut. I am also back to training for my ultras and promoting a low carbohydrate high fat approach for improving metabolic health? How could I have been so wrong? Is a plant based diet good for anyone? Is it carbohydrates or sugar. Is being lean the same thing as being metabolically healthy? I have come to believe it is not. I would like to share a few success stories on how a low carb, high fat diet has helped with glucose control, gastrointestinal symptoms, and triglycerides. It seems crazy that eating a high fat diet would help those things but listen for yourself. Smart people have been arguing for years. People talk a good and convincing game. However, there is a lot of faith and bias involved in nutrition recommendations. Take a listen. It is not always as it seems. Keep an open mind and do your own digging. Jami Dulaney MD Wellness Podcast: Eggs are not to Blame for Your Poor Metabolic Health

Welcome! and Thank you for listening. When it comes to health information, it is really hard to find someone that isn't biased. One way is to be cautious of those making a living selling their own cause. My mission is to help people live longer and healthier. It is my job to continue learning and educating people on the best information I can find. Even if it does not agree with my once held beliefs. Dr Gregor suggested that eggs are bad because of their cholesterol. When you read the sources, it certainly shows that cholesterol is in adipose tissue. And there is inflammation. But it is far from causation and even the study authors admit to not understanding the role cholesterol plays in inflammation. Most of our cells make cholesterol and have cholesterol in the membrane. Surely, we would not possess such a substance if it had no use. Context matter. I will discuss this line of thinking as well as how I use testing to confirm a diagnosis rather than screen without regard for pretest probability. I have adopted a low carbohydrate high saturated fat approach to insulin resistance. I will be looking to improve fasting insulin levels, decrease HgA1C, improve body composition, and decrease inflammation. Using these markers, we will work to dial in members markers to attain optimal metabolic health. If you would like to join us. Jami Dulaney MD Wellness Podcast : Long Term Findings of a Plant Based Diet

Welcome! and Thank you for listening. I practice medicine in a private practice clinical setting. I develop lasting relationships with patients for years. I see their health stats and their struggles over years. I observe their family interactions. This allows me to have a unique view of specific health trends. My practice has been a Plant based wellness practice for 10 years with classes and cooking demonstrations teaching plant based nutrition. We also have weekly and monthly exercise and mobility sessions. Today I would like to share with you what has been happening to folks following or trying to follow a largely unprocessed plant based diet that is 70-80 percent carbohydrate. It is because of these findings, that I have now adopted an animal based low carbohydrate, high fat diet. We will then compare this approach to dietary interventions treating diabetes before and after the availability of injectable insulin. Ultimately, you get to decide what approach might serve you best. We understand the principles and support our patient's choices. Jami Dulaney Wellness Podcast: The Incredible Egg

Welcome! and thank you for listening. There has been some great feedback on a low carbohydrate/high fat diet. Patients are noting weight loss, improved sleep, improved mobility with less joint stiffness and better glucose control to name a few. Things I never thought I would see especially so quickly. However, a there have been some people very disappointed in my direction shift. Many of them had relied on me to convince their spouses that eating plant based was the way to go and now I have pivoted. Others are very uncomfortable facing their friends and answering a whole new set of questions on diet change. My advice would be don't worry be happy. Be happy with the good qualities of your friends and family and they will continue to support you as well. Eggs: are they good or bad? I will present some data on eggs and dementia and eggs and diabetes. You get to choose. However, I am feeling really good about eating my eggs daily. I hope you find the discussion entertaining and thought provoking. Jami Dulaney MD Wellness Podcast: Episode 464: We Still Have Health in Co

Welcome! and Thank you for listening. It has been a week of growth and discussion for sure There has been some early positive results with nutrition shifts to a low carbohydrate high fat diet for many people. There has been improved sleep quality, weight loss, better energy, better glucose control early in their journeys. Some have struggle with low energy levels that have been corrected by changing the types of foods they are consuming. However, there have been anger issues and stones have been thrown. People have felt excluded and lead astray thinking they signed up for a plant based community and now it is gone. What we have in common in this community is a desire to reach optimal health and wellness through nutrition and movement. Just like there are many ways to exercise, there are many ways of achieving optimal nutrition. There are no mandates to change if their current program is working. The only difference is options. In this episode I encourage people to see what they have in common as opposed to focus on differences. Perhaps learn to respect those with a different view or opinion. And if nothing else control the environment around your own feet and project a positive light for others to see. Thanks for listening. Jami Dulaney MD Wellness Podcast Episode 463: Metabolic Science with Dr. Andrew Koutnik

Welcome! And thank you for listening. Today I sit down with Dr. Andrew Koutnik, an award-winning scientist committed to producing and translating cutting-edge research into real world solutions for health, disease management, and performance optimization. Specializing in type 1 diabetes and metabolic health, Andrew has worked with renowned institutions such as Harvard, Johns Hopkins, NASA, and the Department of Defense to develop actionable, evidence-based strategies for overcoming complex health challenges. In this episode we dig into the four-week metabolic adaptation window and why most research misses it, how elevated insulin quietly tanks your energy and recovery even if you think you are eating healthy, what actually happens to muscle glycogen on a ketogenic diet over time, and why the microbiome picture at 6 months looks completely different than it does at week one. We also get into exogenous ketones, continuous glucose monitoring, and why there is no single right diet, just options. Thank you for listening. Jami Dulaney MD Wellness Podcast: Episode 462 -Does Casein Protein Really Cause a Problem?

Welcome! and Thank you for listening. Dr. Richard Feynman, the Nobel Laureate Physicist said, "You must not fool yourself, and you are the easiest to fool" Well then let's get down to business. The Badwater virtual 267mile race is almost completed and I feel great running an average of 16.7 miles a day on a mere 4 grams of carbohydrate. I would never of guessed, but I am quite happy about it. Today I will review in a much different view, The China Study. It was first published in 2006 and I have read and referenced it several times to justify my plant based dairy free diet. I read the book but did not dig deeper until now. I will share the concerns with you on the episode. It is enough to dismiss all of the findings? No. What good came out is that we should eat real food. No processed. Avoid sugar and seed oils. However, the protection from plant proteins are certainly suspect versus animal protein. Listen, learn, read and make your own conclusions. Jami Dulaney MD Wellness Podcast: Continuous Glucose Monitors

Welcome! and Thank you for listening. This episode is about early data of my personal continous glucose monitor. These sensors can be purchased without a prescription and are used to monitor the glucose in the interstitial fluid. Translated there is a mild delay from the blood to the cell fluids where the glucose is monitored and then uploaded to an App on your smartphone. Diabetes and glucose intolerance are the biggest risk factors for cardiovascular disease. Often, people assume that their if glucose is normal fasting, then they are ok. The problem is that insulin resistence occurs much sooner and still carries the risk of chronic insulin elevations. Lifestyle changes can be used to prevent frank diabetes or help one achieve remission. Today I will share with you some of my readings.
