The Six Nails of Tilopa: A Framework for Mental Freedom| HOMe Podcast #031

In this episode of the Health Optimization Medicine Podcast, Boomer Anderson sits down with Dr. Ted Achacoso, Dr. Jup Kuipers, and Dr. Scott Sherr for a different kind of conversation focused on mental health, presence, and the Six Nails of Tilopa. Dr. Achacoso introduces the Six Nails of Tilopa through the mnemonic RITEC R: don't recall, don't imagine, don't think, don't examine, don't control, and relax into effortless awareness. The discussion explores how these principles can be applied to everyday life, particularly when dealing with grudges, future-oriented anxiety, overthinking, perfectionism, and the desire to control outcomes. Join us as we delve into: What Safety Feels Like: How breathing, muscle tension, posture, guarding, and a sense of choice may provide information about how a person is experiencing safety. Six Nails of Tilopa: An introduction to the six principles represented by RITEC R: Don't Recall, Imagine, Think, Examine, Control, and to Relax into effortless awareness. The Sympathetic Hyperdrive: Why the faculty describe modern life as highly stimulating and how chronic sympathetic activation can affect the experience of stress and recovery. HRV and Sympathetic Activation: How Dr. Sherr describes patients who may feel fine while showing persistently low HRV and signs of prolonged sympathetic activation. Past Events and Mental Health: Why learning from the past is different from repeatedly reliving it, and how letting go of grudges can change the way someone experiences the present. Perfectionism and Self-Judgment: How constantly evaluating your own performance can become distracting and exhausting. The Illusion of Control: How the faculty explore the relationship between control, anxiety, decision-making, and the philosophical question of free will. Free Will and the Self: A wide-ranging discussion of whether free will exists and how context, biology, perception, and internal states influence human decisions. The Happiness Game: Dr. Achacoso's practical exercise for noticing how quickly the mind produces another thought. Health Optimization and Presence: Why optimizing health may involve more than quantitative measurements and can also include how fully a person experiences the moment. You are interested in how cellular and nervous-system processes may interact. You have experienced a major stressor or illness and are interested in the recovery process that follows. Learning to Talk Like a Surgeon - Sorena Afshar on Communication in Surgery and Job Interview Prep

How can we better prepare ourselves and our trainees for sitting the oral exam? By this point, residents have typically already passed their written exam. And so they have the book knowledge, but how do you get that book knowledge from inside your brain to your mouth and obvious obviously, onto your examiner and onto their page? To answer that question, I invited Dr. Sorena Afshar onto the podcast this week. Dr. Afshar is a colorectal surgeon in the UK and has a keen interest in communication and surgery. He's also the founder of Surgical Minds, which is a website where you can actually use an AI model to practice your oral exams. We have a fascinating discussion in this episode about language and the nature of learning a new language, what it means to be an immigrant, and how you see your non-native language. But then we really get into the idea of the oral exam, or the Viva. What is the point of the oral exam? How can we best prepare for it? And what are some of Dr. Afshar's tips and tricks for bringing the best of yourself to the oral exam? We also talk about his attempts to use AI to actually examine something quite difficult to tell a computer to understand. Chronic Hand Eczema: A Practical Q&A with the EADV Contact Dermatitis Task Force

Synopsis: In this episode of the EADV Podcast, Dr. Efthymia Soura is joined by Prof. Margarida Gonçalo, Secretary General of the EADV and Head of the Department of Dermatology at the University Hospital of Coimbra, to discuss the Contact Dermatitis Task Force's new paper on chronic hand eczema, written in an accessible question-and-answer format. They cover what defines chronic hand eczema, how to distinguish it from look-alikes such as tinea and psoriasis, and why it functions as an umbrella term covering atopic, irritant and allergic subtypes that often overlap in the same patient. Prof. Gonçalo explains why patch testing (and, where relevant, prick testing) is essential to identify and avoid triggers, and why the different subtypes share a similar molecular signature despite their diverse underlying causes, a finding that for now limits treatment to a largely one-size-fits-all approach. The conversation also addresses the heavy toll chronic hand eczema takes on patients' quality of life, the promise of new topical pan-JAK inhibitors as a possible alternative to steroids, and hope for more personalised, mechanism-based treatments in the future.
